Breathlessness that appears over minutes or hours is completely different from breathlessness that develops over weeks. Sudden means something acute is happening NOW — in the heart or the lung — and it deserves immediate assessment, not waiting "for it to pass".
In short — what to do
- Sit upright — sitting up or leaning slightly forward makes breathing easier. Do not lie down.
- Call 998 if the breathlessness is sudden and severe, and put the phone on speaker.
- Try to slow the breathing: in through the nose, longer out through the mouth — panic worsens the tightness.
- Asthmatic? Use your reliever inhaler immediately as per your plan.
- Loosen tight clothing at the neck and chest, and open a window for cool air.
- Nothing to eat or drink — stay with the person and watch them until the team arrives.

What could be behind it?
- The heart: a heart attack, or fluid building in the lungs (pulmonary oedema).
- The lung: a lung clot (embolism), an acute asthma attack, or a collapsed lung (pneumothorax).
- Severe allergy: breathlessness after food, medicine or a sting — with swelling or rash.
- Telling them apart is the doctor's job — YOUR job: recognise the danger and do the right things until you get there.
What to do until help arrives

- Sudden AND severe breathlessness, or with chest pain.
- Blue lips or face.
- Unable to finish a single sentence in one breath, or confusion.

Sudden breathlessness is not something you "wait out" — the time is now, not later. Every minute counts, and early assessment saves the heart, the lungs and the brain.
